Procedure to open a dental clinic:
Step 1: Prepare documents to open a dental clinic :
Pursuant to Article 43 of Decree 109/2016/ND-CP , the application for an operation license for a specialized dental clinic includes:
a) An application form for an operation license, made according to Form 01 Appendix XI issued together with this Decree;
b) A valid copy of the establishment decision or a document bearing the name of the medical examination and treatment establishment issued by a competent state agency, for state medical examination and treatment establishments, or the business registration certificate of the state agency; business, for private medical examination and treatment establishments, or investment certificates, for foreign-invested medical examination and treatment establishments;
c) A valid copy of the practice certificate of the person in charge of professional and technical expertise of the medical examination and treatment establishment; the person in charge of the specialized section of the medical examination and treatment establishment;
d) List of registered practitioners at medical examination and treatment establishments (including registration of practitioners and medical professionals at the establishment but not subject to the issuance of practice certificates) according to the form prescribed in this Circular. specified in Appendix IV promulgated together with this Decree;
dd) A declaration of physical facilities, medical equipment, organization and personnel of the medical examination and treatment establishment according to Form 02 Annex XI issued together with this Decree;
e) Documents proving that the medical examination and treatment establishment meets the conditions on physical foundations, medical equipment, and personnel organization suitable to the scope of professional activities of one of the prescribed organizational forms. specified in Section 1, Chapter III of this Decree;
g) The charter of organization and operation of state hospitals shall comply with the form prescribed by the Minister of Health; for private hospitals, comply with Form 03, Appendix XI issued together with this Decree and the initial operation plan for hospitals;
h) A valid copy of the contract for transporting patients, for hospitals or maternity wards that do not have means of emergency transport outside the facility;
i) The list of professional and technical expertise proposed by the medical examination and treatment facility on the basis of the list of professional and technical expertise promulgated by the Minister of Health;
Step 2: Submit your application
Location of application: Submit the application at the Department of Health where the clinic is located; or at the Provincial Public Administration Center. For business households setting up a clinic, the application must be submitted to the District People’s Committee.
Step 3: Appraisal of facilities
